Carroll Home Care becomes a part of HomeCare Maryland, LLC

As of October 1, Carroll Home Care has merged with HomeCare Maryland, LLC, a LifeBridge Health affiliate.

Community residents, physicians and business partners will continue to receive the same level of high quality care and customer service previously provided by Carroll Home Care, as well as all the same Associates. The only difference is the name.

Our home healthcare team consists of compassionate and caring nurses, physical therapists, occupational therapists, speech pathologist, home health aides, dietitian, behavioral health, infusion and medical social workers.

All staff members undergo an extensive background check to ensure you receive the highest quality care. Your team of healthcare professionals utilizes an Electronic Health Record to ensure each visiting staff member has quick and easy access to your medical record to collaborate with your physician and other healthcare providers.

New referrals are accepted 24 hours a day, 365 days a year. A nurse is available by phone 24 hours a day. Private duty home care nurses are also available through your long term insurance policy or private pay.

HomeCare Maryland accepts: Medicare, Medicaid, CareFirst, Bravo, Johns Hopkins Connections and Riverside Health insurances.

FAQ/

  • What is Home Health Care?

    Home Health Care is medical treatment for an illness or injury that is given intermittently to the patient in their own home or where they reside temporarily. It cannot be in a Skilled Nursing Facility or Inpatient Rehabilitation Facility. This care is to help you regain your independence and still remain safe and self-sufficient. Home Health offers services such as Skilled Nursing, Home Health Aide, Physical Therapy, Occupational Therapy, Speech Therapy, Dietician, Behavioral Health, Infusion Services and Medical Social Worker.

  • Who is eligible for Home Health Care?

    You must have a physician’s prescription/order for the services. Anyone with a medical condition that confines them to home and requires Skilled Services such as nursing or therapy is eligible. You must not be able to leave your home without the help of an assistive device or the help of another person (Homebound) and your physician must certify this. You may leave your home to go to medical appointments and brief visits to attend church services or the beauty shop.

  • Will there be a care plan customized for my needs?

    Absolutely. Home Care Maryland works with your physician to create a care plan just for your needs. The Nurse or Therapist comes to your home and performs an assessment of your medical condition, your health history and your goals. Then the clinician speaks to your physician to develop your plan of care or treatment. The doctor then signs that order for you. Each week your care team meets to discuss your needs if they changed and to coordinate care among the staff treating you. We contact your physician if your needs or condition change. We will update your plan after consulting with your physician.

  • How often will Home Health visits be?

    The frequency of the visits and the services provided are determined by your physician and your plan of care (as discussed above). Typically home health care is a temporary or intermittent care. Our staff will come for brief visits usually an hour or less a few times per week. Home Health does not typically visit daily as regulated by your insurance carrier. We follow the care plan closely and stay in touch with your doctor.

  • Who pays for Home Health Care services?

    If you meet eligibility requirements, Medicare may pay for the covered home health care as long as you are eligible and your physician certifies that you need it and the doctor orders it. Additionally your state’s Medicaid program or your private insurance may have a covered benefit. Call your carrier directly to see what services you are eligible for and if there any co-pays or co-insurances.

Career Opportunities/

RN Case Manager

Responsible for providing professional nursing care for clinical patients following established standards and practices. Functions as primary clinical liaison between patients and physicians. This position also reports to Service Director for compliance, administrative and operational duties.

Physical Therapist

Coordinates care and provides skilled physical therapy to an assigned group of patients in the home setting. Report to the Service Director or designee.

Occupational Therapist

To perform initial and ongoing patient assessments. To perform skilled occupational therapy functions. To provide supervision and direction to the certified Occupational Therapy Assistant (COTA) providing care in the certified home care. To act as case manager for occupational therapy only cases.
