Physical Therapist

Essential Functions:

• Conducts initial OASIS and ongoing assessment of the physical, psychological and environmental needs of the patient at the start of care, initial PT evaluation and during each visit

• Development and evaluation of the plan of care in partnership with the patient and caregivers

• Providing services that are ordered by the physician as indicated on the plan of care

• Acts as a case manager and determines the needs for other services, notifies Clinical Manager, team members and scheduler of the patient’s needs

• Completes PTA supervisory visits per agency policy/Medicare regulations

• Provides full report to Clinical Manager and appropriate clinical staff to ensure services are provided as needed.

• Completes all documentation per HomeCare Maryland policy, state and Medicare regulations

• Maintains and reviews medical records, provides care based on established plan of care and physician orders

• Communication with all physicians involved in the plan of care and other health care practitioners

• Participate in the QAPI program

• Participation in sponsored in-service training


Supervisory Responsibility:

• Provides supervision to the Physical Therapy Assistant(s) per policy/regulations



• Physical Therapist for the state of Maryland

• Graduate from an accredited Physical Therapy program



• Prior experience in home care preferred, but not required

• 1-3 years acute care experience as a Physical Therapist


Certifications and Licenses:

• Current Maryland Physical Therapy License

• Current American Heart Association BLS Healthcare provider certification

• Valid driver’s license and auto insurance
